School enrollment requirements in Florida

County health departments (CHDs) are responsible for providing recommended or required immunizations to all children birth through 18 years of age and to certain adult clients free of charge. These immunizations are provided along with other routine services or through immunization clinics.
CHD staff also follows up on newborns at risk for being under immunized to ensure that they begin their immunizations. We provide vaccines to children at no cost to the parent/guardian.
The Florida Department of Health in Gilchrist County can issue the form DH 680, Florida Certificate of Immunization or “Blue Card.” This form is required for any child entering Florida schools (public or private, K-12, pre-schools, childcare facilities, or family daycare homes). If your child’s immunizations are not current, we can provide the needed immunizations or try to locate the most up to date information from another provider’s office.
Immunizations are available Monday – Friday from 8 a.m. – 5 p.m.
